Action Learning Team Structure

 

 

The PHC is structured for impact. Our Action Learning Team (ALT) program matches our partners desire to engage in community based challenges with effective facilitation and coordination necessary to be successful. Teams are made up of multiple PHC partners, who have identified an early childhood preventive health challenge that aligns with the PHC’s mission. Each team is paired with an ALTeam Coach who helps them design, develop and implement their project.

Early Childhood Nutrition (ECN) ALT:

Result/Aim: Families with young children (prenatal-5) in Maricopa County provide nutrition food and develop life-long healthy eating habits
Highlights:

  • Planned, coordinated, hosted, and facilitated two stakeholder meetings in the pilot areas
  • Conducted follow up survey of meeting attendees
  • Analyzed small group data for each pilot area
  • Began strategy development
  • Planning follow up stakeholder meetings for November

Isaac School District Community Health Hub (ISD) ALT:

Result/Aim: Develop a Family Resource Center that would serve as a community health hub as part of the Isaac School District. Within one year (by March 2017) complete a comprehensive needs assessment to inform strategic planning and long-term development of the community health hub.
Highlights:

  • The new Grand Opening date is planned for September 2017
  • July 15th is the Family Health Care Day at Isaac Middle School. All community members are welcome regardless of income.
  • The proposal submitted to teh International Industrial Design Association was accepted. The group will be bringing in various contractors for a complete remodel of the Little House.
  • United Health Care is offering Dress for Success classes for promotores with a variety of health topics included and providing professional clothing.

Glendale Strong Family Network (formally Safe Reductions) ALT:

Result/Aim: To reduce Department of Child Safety removals in the zip code 85301 by 10% by 2020.
Highlights:

  • Developed and approved a GSFN governance plan that outlines a leadership, membership and workgroup structure
  • Elected leads for each of the three workgroups (Family Engagement, Partner Engagement, and Communication) and defined a meeting and communication structure for the leads and between workgroups
  • The Communication Workgroup has created a GSFN website (www.glendalestrongfamily.org) that includes a meeting calendar, hosts the online screening survey, and promotes other GSFN member organization events
  • Presented at the Child Abuse Prevention Conference in July, highlighting how the PHC and Safe Reductions Initiative subgroup came together to affect change in Glendale
  • The Family Engagement Workgroup held a focus group in July at the Imes Early Head Start. The group is planning on hosting additional community focus groups in September and October.
    The screening survey began being used by two GSFN member organizations during intake
