Our Partners

The PHC is a broad network of organizations that represent the local public health system, healthcare providers, neighborhood organizations, behavioral health organizations, community based non-profits, early childhood care providers, and private business. The goal is to increase collaboration and capacity within the preventive health system to benefit children birth to five and their families in Maricopa County.

Levels of Engagement

The PHC has introduced a new membership model to make it easier for individuals to get involved in our movement and to make PHC partnership more meaningful to organizational partners. We will begin to distinguish our partners by their level of engagement.

Individuals will now find it easier to join the PHC as “Friends of the PHC” by adhering to our vision and joining us through Facebook, LinkedIn, Twitter and our PHC Newsletter.

“PHC Partners” will be expected to do the above, represent their organization, and attend PHC Meetings and Events.

For those PHC partners who wish to get more involved by participating in Workgroups, Action Learning Teams, and/or the PHC steering Committee will be considered “PHC System Builders.” Lastly, Organizational Partners who agree to actively work towards implementing the PHC Early Childhood Wellness Plan, share their outcome data, and provide the collaborative with insights and lessons learned will be “PHC Organizational Partner systems Builders.”

  • Can represent their organization but is not required to.

PHC Systems Builder - Partner

  • Represents the PHC by wearing the PHC Systems Builder Pin
  • Is a member of the Steering Committee, ALT or Workgroup
  • Submitted signed Partner Agreement Form and Represents their organization

PHC Systems Builder – Organization

  • Submitted signed Partner Agreement Form
  • Agrees to improve PHC Early Childhood Wellness Plan indicators.
  • Delegates one point-person from their organization
  • Shares Outcome Data
  • Shares lessons-learned with the collaborative
